Becoming a Patient

To sign on and register with our family physicians to become a patient, we encourage you to call our clinic to schedule an initial meet and greet appointment where you and the physician can get acquainted. After that you are welcome to register with the doctor to become a patient. To see our physician you call our
clinic at 905-857-9797 to schedule an appointment.

Home Visits

Our family physicians do home visits for registered patients who are housebound or unable to physically attend the clinic due to their medical condition. To request a home visit you have to call the clinic to schedule an appointment with our physician.

How can I communicate with my physician?

Our physicians and clinic do not respond to personal emails, text messages etc. These means of communication are not secured. We request that you make an appointment to visit with your physician at the clinic to discuss any concerns.

How should I follow- up regarding my test results?

We endeavour to contact our patients to follow up on their results once received and reviewed by the physician. Never assume no call is good news. In the event that you are not contacted for a follow-up appointment within two weeks of completing your test, please call us to book a follow-up with your doctor.

Can I get my test results over the phone?

Test results will NOT be provided over the phone by any of our office staff and the doctor will not come to the phone at your request to discuss the results. Please book a follow-up appointment to review and discuss your results with the doctor in the privacy of the clinic.

Cancellation Policy

To cancel or reschedule a booked appointment you have to do so in person or by phone to a live person during business hours. Answering machine messages will not be accepted. Same day booked appointments are considered confirmed and may be subject to a cancellation fee.

Prescription Refills

Due to privacy concerns we are unable to fax or phone prescription repeats to your pharmacy. Do call our office to schedule an appointment for your next refill before you run out of your medication.

Third Party Insured Services and Uninsured Medical Services

Not all Medical services are insured by OHIP. Examples of uninsured services include sick notes, insurance forms, government forms, employment physicals, travel health, tray fees for procedures, etc. Forms take time to complete and thus reduce the doctors ability to provide care to patients. We will always inform patients in advance of such charges and offer several convenient forms of payment. All charges for uninsured services must be settled at point of service.

OHIP Policy

A valid health card is required for all OHIP visits. An invalid or expired health card will attract a fee of $50. Fee may be reimbursed if proper valid documentation is provided in a timely manner. Out of province patients are not covered by OHIP, official receipts are provided for reimbursement purposes.
